Children attending our School Holiday Program who have a medical condition are required to complete a risk minimisation plan and medical action plan.
All medical forms must be received before the child attends our School Holiday Program. Forms should be emailed to enquires@brightonrec.com.au.
1. Enrolment Form
To enrol in our School Holiday program in 2021, you will first need to complete a 2021 Enrolment Form. Please ensure the document title contains your family name.
This enrolment form only needs to be completed once each year and can be completed at any time prior to bookings opening. Please email the form to enquiries@brightonrec.com.au.
2. Risk Minimisation Plan
Children with a medical condition must complete a Risk Minimisation Plan.
3. Action Plan
If you child has a medical condition, please complete a condition specific action plan or the general action plan below. Action plans must include a picture of the child.
